Ink Color - Fill in the box(es) next to your ink color(s) choice. For products with two or more colors, indicate the ink color for each line and graphic in the personal information section. All products will be printed in thermography (raised printing) unless offset (flat printing) is indicated. |

Special Handling - When ordering flat printing, lasersafe thermography or when requesting a direct shipment, check the appropriate box. |

Typestyle - Fill in the mainline typestyle and body copy typestyle in the boxes provided. Fill in the box next to "as shown in format Design Idea" if you would like the typestyles shown in your chosen format Design Idea. |

Graphics - Fill in the graphic number you have chosen on the line provided if your custom or format Design Idea includes one. Fill in the box if you are attaching your own artwork. |

Personal Information - For Design Ideas use the lines that correspond with the line numbers on your chosen Design Idea. For Custom Layouts use the lines as a guide and enter your information in the approximate position. |

Foil Color - When ordering foil stamped products, fill in the box(es) next to your foil choice(s). For products with 2 or more foil and ink colors, indicate the foil or ink color for each line and graphic in the personal information section. |
